At Involvement Support, every service begins with a clear and structured assessment. We do not provide generic packages or pre-set solutions. Support is developed through understanding the individual, their environment, their strengths and the risks that may be present.
An assessment-led approach ensures that care is proportionate, safe and aligned with agreed outcomes from the outset.
Effective support starts with clarity. Without a thorough understanding of needs, routines and risks, support can become reactive or inconsistent. Our assessment process helps us identify strengths and capabilities, understand behavioural and emotional triggers, clarify communication needs and assess environmental and contextual factors. It also allows us to identify safeguarding concerns, agree measurable outcomes and determine appropriate staffing levels, ensuring support is built on observation and evidence rather than assumption.
Each assessment is tailored to the individual. It may consider personal history and current presentation, health and wellbeing needs, behavioural patterns and known triggers, and the views of families and professionals. We also consider risk and safeguarding, education or employment needs, and how the person experiences community life. Where relevant, we take account of cultural, spiritual and personal identity considerations, and we focus on what matters most to the person by agreeing desired short and long-term outcomes. Throughout this process, we work collaboratively with families, social workers, health professionals and other relevant stakeholders.
Assessment findings inform the practical plan for delivery. This includes the support plan itself, risk management arrangements, positive behaviour strategies where required, and a staffing model appropriate to need. Plans are written clearly so that everyone involved understands the goals, the agreed approaches and what good support should look like day to day. A review schedule is agreed so progress can be tracked and support can evolve as needs change.
Assessment is not a one-time event. People’s circumstances develop, risks change, and progress can create new goals. We review support regularly to ensure risks remain appropriately managed, outcomes are being progressed, support levels remain proportionate and strategies remain effective. Where adjustments are needed, we respond promptly and work in partnership to keep support safe, structured and sustainable.

Our aim is always to stabilise situations, strengthen foundations and support progress towards greater independence over time.
